When they render local or mobile telephone services.

Free form document, delivered in the Filing Office of the Federal Telecommunications Institute or, by means of electronic media, delivering a compact disc or memory device including the corresponding notice.

The free form document shall be accompanied by the storage electronic media (CD) including the map in Arcview (.shp, .dbf and .shx), Mapinfo (.tab) or AutoCAD (.dxf) format.
